What Kate Did
Of all the shocks in this episode, one surprised me more than the obvious ones. In Kate's flash-sideways she takes Claire to the hospital and her doctor is Ethan! Weird, right? But what was really odd is that he introduces himself as "Dr. Goodspeed," On the island, we knew him as Ethan Romm. Goodspeed was killed by Ana Lucia in Season 2: The Other 48 Days. So my question: why is there a name-switch? I'm intrigued by many of the differences we've witnessed in this alternate universe of 2004, but this one confounds me for its difference.

All Figured Out
I've finally got enough to understand the show: Much like the Grey's/Private Practice Cross Overs, the writers are attempting a similar strategy - the title LA X implies this. Hear me out:
Greg Grunberg, who plays the Oceanic 815 captain (aka Matt Parkman), used his mind control abilities to make all the passengers believe they crashed on the island. In doing so, he is protecting them from the Man in Black (aka Sylar) who wants to steal their powers (ex: Hurley's Talking to Ghosts, Kate's ability to refrain from commitment). By setting off Jughead, they created an "incident" which freed part of their consciousness. Now the quest will be for Sawyer to find a pair of horn-rimmed glasses (HRG) to read "Turn of the Screw" and discover the means to save the cheerleader to save the world. Clearly, that job will be for Desmond - he's special, darn it!
